form, we ask for your first name and e-mail address (first name used solely
to personalize requested information). When purchasing a store product or
service, additional mailing/shipping information as well as payment information
is required.

and fulfill orders placed at the Motivation123 Web site.

This link can also be used to terminate your subscription with Motivation123
and remove your personal information from our system. To do so, click
the subscriber options link and change your subscription status
accordingly.
You can also change or terminate your subscription by sending a request
using our online contact form. We will personally update our records and
confirm the changes when completed.
